| How to Request an Official Transcript |

| 1. Complete the Transcript Request Form (attached at the bottom of this page) |
|
• Download the PDF or pick up the Official Transcript Request Form from the High School Office
• Provide the following information:
- Full name at the time of attendance
- Date of birth
- Date you are submitting the request form
- Year of graduation
- Current contact information

| 2. Provide Recipient Information |
|
|
• Clearly indicate where the transcript should be sent.
• Include the full mailing address or secure email address.
|
|
|
|
|
| 3. Authorization Requirement |
|
|
• A signed request is required by law before transcripts can be released.
• Requests without a signature will not be processed. |
|
|
|
|
| 4. Processing Time |
|
|
• Please allow up to 10 business days for processing.
• Extra time may be required during busy periods (college application season, beginning/end of school year, summer, etc.).
• Transcript requests must be submitted at least 10 business days prior to any deadlines |

| 5. Delivery Options |
|
|
• Transcripts can be:
- Mailed directly to the requested institution
- Sent electronically directly to the requested institution
• Reminder: Transcripts are considered official only if sent directly by the high school or provided in a sealed, unopened envelope. |
